The Indianapolis Fire Department is committed to building a safer, stronger, and more resilient organization through transparent review of significant incidents. Every event\u2014whether routine, complex, or high-risk\u2014serves as an opportunity to identify strengths, address hazards, and advance operational excellence for our members and the community we serve.<\/p>\n\n\n\n

To support this mission, the department utilizes two formal review processes:<\/p>\n\n\n\n

Post-Incident Analysis (PIA):<\/strong> 
A structured review is conducted with the companies that operated on the scene. The PIA examines tactics, communication, decision-making, and operational considerations to highlight what went well, surface opportunities for improvement, and reinforce effective practices.<\/p>\n<\/div>\n\n\n\n

Significant Incident Investigation Team (SIIT):<\/strong> 
A comprehensive investigation is conducted following serious events such as firefighter injuries, serious bodily injury, critical incidents, or line-of-duty deaths. SIIT reports identify root causes, contributing factors, and actionable recommendations for policy, training, safety, and operational change. These investigations support organizational learning and ensure that lessons from serious incidents drive meaningful improvement across the department.<\/p>\n<\/div>\n<\/div>\n\n\n\n

Special Reports:<\/strong>
Special Reports are created when an incident or issue falls outside routine operations and requires deeper analysis. These may follow specialty operations events, mutual-aid responses, disaster deployments, or any situation not addressed through standard investigative pathways. Special Reports can also include research briefs or white papers on firefighter health, emerging hazards, or trends within the fire service. Their purpose is to document key findings, identify contributing factors, and provide evidence-based recommendations that strengthen policy, training, and operational readiness while supporting continuous improvement across the department.<\/p>\n<\/div>\n<\/div>\n\n\n\n

Together, these reviews reinforce a culture of safety, accountability, and continuous improvement at every level of IFD.<\/p>\n\n\n\n
